concealer/foundation help?

ok so i have a problem.....i use mac concealer and foundation...and when i put on my concealer it covers everything up...but when i put the foundation on my face it seems like the blemishes appear again..idk...maybe im rubbing everything off when i put the foundation on....idk....and i also heard that you are supposed to put your concealer on after the foundation...help please
