Default Foundation and concealer doesn't stay on my face?

ok i wear make-up almost every day and what i do is i wash my face with cleanser and then apply concealer, then foundation, then eye make-up

but as soon as i go outside, i go really pale because the foundation has sort of washed off

it mainly comes off my cheeks and under my eyes

Should i try powder? what is the best powder to use? (i dont want it to look cakey)
i use RIMMEL concealer and MAYBELLINE foundation
could this be the prob?
btw its liquid foundation ive been using and stick concealer! thanks
