concealer usually comes in a stick form, so put that on first and blend it in with your finger. then, assuming you have liquid foundation, use either your finger or a makeup sponge to put it on.
also, if you go to neutrogena.com or loreal.com they have videos on how to apply makeup.

U use your fingers to rub the foundation in and wipe sides of face so u will not have a line around face.

its a brush if you have one... if not a finger will work
